The conception of saviour siblings using preimplantation genetic diagnosis coupled with human leukocyte antigen typing or HLA typing alone is controversial and receives a wide divergence of legal responses among countries around the world. The resulting child conceived through this procedure is dubbed a ‘saviour sibling’ as the child can potentially act as a compatible donor for an elder ailing sibling who needs a haematopoietic stem cell transplantation. At present, the acceptability of this procedure in Malaysia is ambiguous as there (...) is no specific statute governing assisted reproductive technology, and the guideline issued by the Malaysian Medical Council does not expressly address this issue. The ethical arguments relating to the conception of saviour siblings in the current literature are mainly thought to reflect a liberal viewpoint which is predominantly Western in nature. This paper seeks to explore the perception of selected key stakeholders on the acceptability of and ethical concerns related to this procedure in the Malaysian context. The findings indicate that stakeholders generally support the conception of saviour siblings and some of their concerns echo the Western viewpoint. Interestingly, many were highly anxious about its economic implication, which is rarely discussed in the current literature on saviour siblings. (shrink)

The occurrence of a new strain of SARS-CoV-2 cannot be ruled out. Therefore, this study seeks to investigate the possible effects of a hypothetical imperfect anti-COVID-19 vaccine on the control of not only the first variant of SARS-CoV-2 but also the second variant of SARS-CoV-2. We further examine the rates and a, escape of quarantined infectious individuals from isolation centers. The control R c and basic reproduction numbers R 0 are computed which gives assess to obtain asymptotic stability of disease-free (...) equilibrium point globally and the existence of a unique persistent equilibrium solution. Numerical results reveal that people infected with the second strain who are vaccinated with an imperfect vaccine are under control but the prevalence of the second variant enhances the prevalence of the first variant. Thus, discovering a vaccine that is effective for the prevention of variant 2 is necessary for the control of COVID-19. Numerical results also reveal that increase in the rate at which individuals infected with the first variant escape the isolation center gives rise to the population infected with the first variant and lowers the peak of the population infected with the second variant. This is probably because individuals infected with the second variant appear to be more careful with their lives and get vaccinated more than individuals infected with the first variant. (shrink)

This study examined the association between employee perceptions of two foci of corporate social responsibility and work attitudes in different countries. Using data collected as part of a multinational research project with a core team in the United States, we found that perceptions of externally focused CSR enactment were positively associated with employee engagement and affective commitment. Perceptions of internally focused CSR enactment were positively associated with affective commitment but not with employee engagement. Analyses across countries revealed more cultural than (...) economic differences. For example, perceptions of internally focused CSR enactment were consistently associated with affective commitment across cultural contexts, indicating that they might serve as a general foundation for building commitment. Perceptions of externally focused CSR were more strongly associated with affective commitment in Anglo than in Confucian and Latin American countries, suggesting a need for country-specific tailoring. Based on these results, we provided recommendations for planning and implementation of CSR. (shrink)

Jamgön Kongtrul's encyclopedic Treasury of Knowledge presents a complete account of the major lines of thought and practice that comprise Tibetan Buddhism. Among the ten books that make up this tour de force, Book Six is by far the longest—concisely summarizing the theoretical fields of knowledge to be studied prior to the cultivation of reflection and discriminative awareness. The first two parts of Book Six, contained in this volume, respectively concern Indo-Tibetan classical learning and Buddhist phenomenology. The former analyzes the (...) traditional subjects of phonology and Sanskrit grammar, logic, fine art, and medicine, along with astrology, poetics, prosody, synonymics, and dramaturgy. The principal non-Buddhist philosophical systems of ancient India are then summarized and contrasted with the hierarchical meditative concentrations and formless absorptions through which the “summit of cyclic existence” can genuinely be attained. Farmers’ decisions to conserve natural resources generally and soil and water particularly are largely determined by their knowledge of the problems and perceived benefits of conservation. In Ethiopia, however, farmer perceptions of erosion problems and farmer conservation practices have received little analysis or use in conservation planning. This research examines farmers’ views of erosion problems and their conservation knowledge and practices in the Beressa watershed in the central highlands of Ethiopia. Data were obtained from a survey of 147 farm households (...) managing 713 fields during the 2002/2003 cropping season. In-depth interviews and group discussions were also held with the farmers to obtain additional information. The results show that 72% of the farmers reported erosion problems, and they recognized that conservation was necessary. However, they considered erosion to be severe mostly when visible signs – rills and gullies – appeared on their fields. The majority of the farmers believe that erosion could be halted, and they use a range of practices for erosion control and fertility improvement. These include contour plowing (83%), drainage ditches (82%), and stone terraces/bunds (73%). Nevertheless, despite decades of conservation intervention in the area, it appears that most farmers have developed negative attitudes towards externally recommended measures. The research concludes that under the conditions present in the Ethiopian central highlands, soil and water conservation interventions should consider farmers’ conservation knowledge and practices to improve acceptance and adoption of the recommendations. Background The use of lengthy, detailed, and complex informed consent forms is of paramount concern in biomedical research as it may not truly promote the rights and interests of research participants. The extent of information in ICFs has been the subject of debates for decades; however, no clear guidance is given. Thus, the objective of this study was to determine the perspectives of research participants about the type and extent of information they need when they are invited to participate in (...) biomedical research. Methods This multi-center, cross-sectional, descriptive survey was conducted at 54 study sites in seven Asia-Pacific countries. A modified Likert-scale questionnaire was used to determine the importance of each element in the ICF among research participants of a biomedical study, with an anchored rating scale from 1 to 5. Results Of the 2484 questionnaires distributed, 2113 were returned. The majority of respondents considered most elements required in the ICF to be ‘moderately important’ to ‘very important’ for their decision making. Major foreseeable risk, direct benefit, and common adverse effects of the intervention were considered to be of most concerned elements in the ICF. Conclusions Research participants would like to be informed of the ICF elements required by ethical guidelines and regulations; however, the importance of each element varied, e.g., risk and benefit associated with research participants were considered to be more important than the general nature or technical details of research. Using a participant-oriented approach by providing more details of the participant-interested elements while avoiding unnecessarily lengthy details of other less important elements would enhance the quality of the ICF. Background Informed consent is a process in which a healthcare provider obtains permission from an individual prior to surgery. Patient satisfaction with the informed consent process is one of the main indicators of healthcare service quality. This study aimed to assess patient satisfaction with surgical informed consent at Jimma Medical Center, Ethiopia, in 2020. Methods A facility-based cross-sectional study was conducted from April 1 to June 30, 2020, at Jimma Medical Center. Face-to-face interviews were conducted using structured questionnaires. A systematic (...) sampling technique was used to select the study participants. The collected data were coded, entered into Epi data version 3.1, and analyzed using SPSS version 25. Bivariate and multivariate regression analyses were performed to determine the association between patient satisfaction and socio-demographic and facility-related factors. In multivariate regression, predictors with a P-value of < 0.05 were considered statistically significant. Results Totally 372 study participants were interviewed with a response rate of 97.8%. Nearly two-fifths (43%) of patients were satisfied with surgical informed consent. Living in an urban area (AOR: 2.279, 95% CI 1.257–4.131), having current referred history (AOR: 1.856, 95% CI 1.033–3.337), consent form version (AOR: 2.076, 95% CI 1.143–3.773), time spent on the provision of informed consent (AOR: 5.227, 95% CI 2.499–10.936) and having better patient-health providers relationship (AOR: 5.419, 95% CI 3.103–9.464) predictors were positively associated with patient satisfaction. Conclusion Patient satisfaction with the surgical informed consent process was relatively low. Therefore, Health care professionals need to emphasize a way of delivering informed consent, patients' needs and obey a standard informed consent to improve patient satisfaction. (shrink)

Hong Kong is undergoing two overlapping crises: social unrest over anti-government protests, and COVID-19. The media has linked these events in both objective and subjective ways. While some liken the social unrest to COVID-19, others do the opposite. This is an intriguing real-world instance of source-target reversibility with interchangeable source and target resulting in two apt variants. This paper reports a survey study of the links between crisis perceptions and the aptness of metaphor variants. Participants (N = 93) rated 30 (...) matched items on the effects of both crises on trust in governance, interpersonal relations, the economy, physical/mental health, and Hong Kong’s future. This determined, for each participant, a correlation coefficient reflecting perceived structural similarity, and absolute/raw difference scores reflecting perceived substantive similarity of the crises. They then explained which (or neither) of two constructed headlines depicting the SOCIAL UNREST as COVID-19 or COVID-19 as the SOCIAL UNRESTwas more apt. Logistic regression analyses showed that i) metaphor aptness was predictable from structural but not substantive similarity, and ii) the worse crisis was preferred as source domain, but only for its more experientially concrete aspects. It is assumed that bipolarity in happiness and sadness requires mutual exclusion. However, we present psychometric research to show how coendorsements of happiness and sadness do not necessarily constitute evidence against bipolarity. Because individuals have a tendency to endorse emotion terms close to their current state, individuals whose current state is close to the middle of a bipolar continuum would report both happiness and sadness, despite their current state being best represented by a single point. As such, endorsements of happiness (...) and sadness are theoretically expected when individuals’ current state is in the middle of the continuum. Bipolarity however would be violated if endorsements of extreme levels of happiness and sadness can be demonstrated, which has yet to be shown. (shrink)

This article explores the interfaces between two constructs in linguistics and psychotherapy—metaphor and the human body—as a means of illustrating meaningful exchange between linguistic and mental health research. Three distinctly well-motivated research strands with underexplored overlaps: the theoretical relationship between metaphor and the body, the use and management of metaphors in therapy, and the body as a therapeutic resource complementing verbal interaction, are first described. Taking a practitioner-informed approach called “correspondent analysis,” which combines the methods and insights of the metaphor (...) researcher and psychotherapist, a series of session extracts are then analyzed to illustrate the connections between the three strands and what they imply for both metaphor research and therapeutic practice. This culminates in the proposal of a triangle model for the metaphor–body–psychotherapy relationship, with wider implications for applied metaphor research also discussed. (shrink)

John Dewey relates ethics in general with the mode of inquiry. Against the mainstream ethics and moral theories, Dewey reconstructed morality in light of empirical science, providing the necessary steps of pragmatic ethical investigations. In this study, I have revisited Dewey’s ethical inquiry and recent developments of the methods of pragmatist bioethics. Using this approach in ethics, I have examined the development of reproductive technologies and genetics, precisely the moral dilemma of gestational surrogacy at the level of a public issue (...) that needs social policy. In the final part, I have suggested the significance of Dewey’s emphasis on education, deliberative democracy, and institutions and agents’ role as the basis to solve bioethical issues arising in different societal contexts. Time series analysis is a technique to describe the structure and forecast values of a particular variable based on a series of sequential observations. While commonly used in finance and engineering to understand structural changes across time, its applicability to humanistic processes like discourse is less clear. This article demonstrates the feasibility and complementary use of TSA with a case study of metaphor use in psychotherapy sessions. A conceptual sketch of how TSA components relate to discourse components is followed by (...) a step-by-step analysis of two therapist–client dyads. Each dyad is shown to be adequately described by a different model, and the key characteristics of the models are interpreted alongside prior qualitative analysis of the dyads, shedding new light on their structural properties and attendant implications. This superb collection of writings on buddha nature by the Third Karmapa Rangjung Dorje (1284-1339) focuses on the transition from ordinary deluded consciousness to enlightened wisdom, the characteristics of buddhahood, and a buddha's enlightened activity. Most of these materials have never been translated comprehensively. The Third Karmapa's unique and well-balanced view synthesizes Yogacara Madhyamaka and the classical teachings on buddha nature. Rangjung Dorje not only shows that these teachings do not contradict each other but also that they supplement each other (...) and share the same essential points in terms of the ultimate nature of mind and all phenomena. His fusion is remarkable because it clearly builds on Indian predecessors and precedes the later often highly charged debates in Tibet about the views of Rangtong ("self-empty") and Shentong ("other-empty"). Although Rangjung Dorje is widely regarded as one of the major proponents of the Tibetan Shentong tradition (some even consider him its founder), this book shows how his views differ from the Shentong tradition as understood by Dolpopa Taranatha and the First Jamgon Kongtrul. The Third Karmapa's view is more accurately described as one in which the two categories of rangtong and shentong are not regarded as mutually exclusive but are combined in a creative synthesis. For those practicing the sutrayana and the vajrayana in the Kagyu tradition, what these texts describe can be transformed into living experience. (shrink)

Shaping off-job life is becoming increasingly important for workers to increase and maintain their optimal functioning. Proactively shaping the job domain has been extensively studied, but crafting in the off-job domain has received markedly less research attention. Based on the Integrative Needs Model of Crafting, needs-based off-job crafting is defined as workers’ proactive and self-initiated changes in their off-job lives, which target psychological needs satisfaction. Off-job crafting is posited as a possible means for workers to fulfill their needs and enhance (...) well-being and performance over time. We developed a new scale to measure off-job crafting and examined its relationships to optimal functioning in different work contexts in different regions around the world. Furthermore, we examined the criterion, convergent, incremental, discriminant, and structural validity evidence of the Needs-based Off-job Crafting Scale using multiple methods. The results showed that off-job crafting was related to optimal functioning over time, especially in the off-job domain but also in the job domain. Moreover, the novel off-job crafting scale had good convergent and discriminant validity, internal consistency, and test–retest reliability. To conclude, our series of studies in various countries show that off-job crafting can enhance optimal functioning in different life domains and support people in performing their duties sustainably. Therefore, shaping off-job life may be beneficial in an intensified and continually changing and challenging working life. (shrink)

Using data from user comments to the official social networking account of the Hubei Red Cross Foundation on a participatory web platform, this study attends to the offensive and hateful comments produced by ordinary Internet users to blame the elite authorities for their malfeasance in managing the donation during the COVID-19 in China. Drawing on Discursive Psychology, we focus on the rhetorical strategies that users employ to legitimise their actions as well-founded evidential blame against a norm-breaking act rather than radical (...) extremist speech. The associated hatred among discussants are moral, social judgements. That said, hate speech also helps construct the moral standards of a normalised society. (shrink)

The fundamental belief in destiny in Yoruba culture is explained within the tradition that for every individual person who comes to aye (earth), there is a package of destiny containing the totality of all that such person will be. However, the content of this destiny is not known to any person except Orunmila, one of the deities. Therefore, it is believed that a person dies if and when he/she has exhausted the content of his/her ori (package of destiny). Included also (...) in the Yoruba belief system is that a youthful death is a sorrowful death. This is predicated on the premise that a young man could not have completed the content of his earthly mission. His death is therefore sorrowful, and he could therefore not be admitted into Orun(“heaven”) to join the league of the ancestors. This is the explanation for the belief in reincarnation, and, more specifically, the belief in akudaaya or abarameji(reincarnated persons).This paper argues for two main points. First, in spite of the Yoruba belief that no human being is privy to the content of destiny, the Yoruba belief in sorrowful or sudden death (ikuofo or iku-ojiji) presupposes some knowledge of the content of each person’s destiny. Second, the beliefs in destiny on one hand, and in sudden or sorrowful death on the other, are mutually exclusive. This means that if one is true the other will be false and vice-versa, so that holding the two together would be contradictory. Because such contradictions are common in the entire gamut of Yoruba belief, there is need for a critical examination of theYoruba belief system in the quest for a modern culture. (shrink)

Although much has been written about the use of metaphors during psychotherapy sessions, the complementary question of how the therapeutic process might itself be metaphorically conceptualized is seldom asked. This article adopts the notion of ‘discourse metaphors’ and provides a case study of the metaphor THERAPY IS A JOURNEY across various levels of psychotherapeutic discourse, including the formulation of theoretical constructs, pedagogical frameworks and transcripts of actual therapeutic talk. I show how the inherent meaning stability as well as flexibility afforded (...) by conceptual metaphors renders them particularly suitable for conveying a necessary sense of theoretical continuity across the levels, while accommodating variations according to situated discourse objectives within each level. Answering calls for metaphor research to be more practically relevant, I also suggest how the present approach can contribute to the increasing therapeutic concern with establishing ‘feedback’ across the different participatory levels of psychotherapy. There is a prevalent assumption that the phenomenal character of a mental experience is an ontological property existing as part of the fabric of the world. This implies that the problem of explaining the phenomenal property of a mental experience is a metaphysical one. Contrary to this assumption, the present paper argues that phenomenal properties of mental experiences are the results of our epistemological perspectives of the world. This article focuses on examining the situation of organ trafficking in Africa from the aspect of pragmatist ethics. In the mainstream thought, the broader ethical dilemma of organ trafficking is viewed within the moral contestation of altruism as a rule for organ procurement and the resulting worldwide organ shortage. The incapability of altruistic transplant orthodoxy to serve as an applicable foundation for a public policy is considered as a reason for organ trafficking. In fact, to battle organ trafficking, utilitarian-inclined studies (...) suggest organ selling, compensated donations, and non-directed paid donations as practical alternatives. However, when investigating the context of organ trafficking in Africa, the issue goes beyond the mere moral dilemma of altruism and organ shortage. In the region, organ trafficking is rooted within more systemic, structural, socio-economic, and political problems, grounded in the abuse of transplantation, and connected to transplant tourism and migration, which needs a pragmatic multimodal solutions than the earlier mainstream pathways. In pragmatist ethics, the moral assessment and solutions to the underlying problems depend on the context wherein the moral problem arises. Pragmatist ethics does not stick to absolute moral theories; instead, it focuses on the relevant assessment of the felt moral problem in a given context and aims at seeking solutions afresh. Thus, in the subsequent sections, this article looks at the context of organ trafficking in Africa and reassesses it by incorporating it into the ethical debates of transplantation and organ procurements and trading. The final part of this article reflects on solutions with the reconsideration of the context of transplantation and organ trafficking in the region. (shrink)

The qualitative nature of co-deployed or “associated” metaphorical source domains in discourse has been extensively researched, often in terms of whether they share common conceptual roots. There are however few empirical studies on the strength of these associations and their implications. In mental healthcare activities like psychological interviews and counseling, for example, strongly associated sources may suggest unique “conceptual assemblages” that highlight underexplored (dis)similarities between clients or client groups, beyond the typical focus on isolated sources and frequencies. Our case study (...) compares source domain associations produced by two interviewee groups – one meeting the diagnostic threshold for acute stress disorder (ASD), the other not – relating their first-hand experiences of traumatic events during the 2019 Hong Kong social unrest. The machine learning method of association rule mining is used to extract and quantify the top association rules in both groups. Results show i) a shared rule suggesting a highly schematic construal of trauma that nevertheless varies in instantiating details, and ii) several distinct rules that corroborates and lends further insight into the “agentive vs. non-agentive” characteristics exhibited by ASD vs. non-ASD individuals. In the Qur'ān, a mysterious event is mentioned in four different parts. According to the most mufassar (exegetes), the essence of this event in the Qur’ānic verses is the knowledge of revelation, which include divine orders about how the earthis transferred to the lower level, starting from the angels in the upper level. Ultimately, this information is delivered to the missionary angel who will execute the order. During this transfer, the devils secretly try to capture this information. The purpose of (...) Satan is inform the priests to inform them in advance. However, shihābsstep in and prevent the devils to reach these information. Thus, the security of the information, which include divine orders, are provided and prevented the passageof unauthorized hand. This event in the Qur'ān requires a systematic analysis of the culltural structure of pre-Islamic Arab society, the perception of the incident of the interlocutory mass in the process of conquest, and the various comprehension efforts in parallel with the cultural changes that have taken place. The problem of preserving the knowledge of unseen with shihābswill be studied in various aspects by considiring the subject and taking into consideration the different approaches in the historical process.SummaryThe Qur’ānic verses are classified in two kinds such as mutashabih (ambiguous) end muhkam (unambigious) according to the meaning of the text. The verses that convey the meaning immediately without recourse to any further analyses, are called “muhkam”, and the verses that appear to be ambigious meaning in terms of signifying more than one possible meanings, are called “mutashabih”. One of the mutashabih which mentioned in the verses is prevention of the attempts of the demons which trying to steal unseen knowledge with shihābs. The attempts of the demons to listen to the unseen realm were included in different surahs. This subject fall into the verses of 40/al-Jinn 72/8-10, 54/al-Hijr 15/16-18, 56/al-Saffat 37/6-10 ve 77/al-Mulk 67/5 depends on their occasion of the revelation of the Qur’ān order. In these verses, the concepts of shihāb, mārid, rajim, duhūr, malaa’l-a’lā, haresen shedīden, man khataf al-khatf ve burūc come to the forefront and the various linguistic analyzes are made about them.Shihābs, which thrown to prevent the demons from listening to the sky did contention as a part of prophet Muhammad in risaleti and various ideas were put forward in this direction. In addition to those who acknowledged this event as a miracle of apostleship, and who acknowledged that they emerged together with apostleship, opinions were also put forwerd based on before apostleship by attributing various evidence. All these opinions are explained in detail.In the first period, the preservation of the sky with shihābs was understood as a physical fact and various inferences were made in this direction. It is possible to see this situation clearly in the riwayats which are based on prophet Muhammad. According to this, shihāb is nothing else from preserving preserving the sky by moving fireballs. In some comments, the star shift is explained as the shihāb.As in other matters, the tredition of the isharī tafsīr has been approached this event with a different perspective. These moving parts of fire, seen in the outer realm in the conservation of divine knowledge, are expressed in terms of concepts that will represent the struggle of the human commentators against the devil in the inner world of human. According to them, the shihābs are the fire of spiritual knowledge that burns the devils. In this way, human fend off kind of bad thoughts that surround himself in his spiritual world.The shihābs was regarded as the miracle oh prophethood, because it was being used as a guard mechanism to prevent the divine order coming to the Prophet Muhammad from falling into the hands of devil forces. According to this, it was claimed that the shihābs first appeared together with the apostleship. However, it was documented by some scholars were based apostleship because of the various examples of poetry belonging to the jahiliyya period. Whether it is the timing of the event or the views that base it before or after the apostleship, the point in which they all converge is that the movement of sky in the occasion of the revelation of the Qur’ān period has increased more markedly than the other periots. At the same time, the inability of bringing reports of devils who were the source of the knowledge of priests was accepted as a reason for the disappearance of the job soothsayer which was common in Arab society. Throwing of shihābs and as a result of this disappearence of priesthood were accepted as the miracle of the prophet Muhammad.In the tafsir tradition, shihābs were accepted as the stars thrown into the demons. However, it was observed that there was a different approach to this incident in the modern period. The prominent idea is that this incident which described in the verses is a matter of concern to unseen realm. Therefore, it was emphasized that things which they said about the subject could not go beyond the predictions. The idea of being accepted without any additions to the expressions of the verses came to the forefront.One of the ideas raised recently is the level of perception of unseen realm According to this, the verses do not mention any obstruction in the outer orgy. As a claim that, to speak of fireballs and moving stars is opposite to the universe order. This information subject to the verses is nothing but the thought of perception of revelation in Arab society. This article approaches the ethical dilemma of healthcare allocation and rationing from the perspective of pragmatist ethics, mainly following John Dewey’s ethics. The moral dilemma of healthcare allocation arises whenever we allocate limited resources, and rationing is a necessary option for distributing available resources. In a broader sense, the moral problems of healthcare allocation also encompass the issue of access to primary healthcare, especially for low-income sections of communities. In this sense, allocation always entails rationing – denying service to someone (...) for the benefit of others. Such aspects of allocation and rationing and the relational aspect of disease and health make the problem morally controversial, which makes it difficult to agree on a principle or principles of allocation and rationing applicable across different contexts. Hence, this paper argues that moral challenges of healthcare rationing ought not to be addressed through the appeal to principles, but rather through deliberation that embraces a more pragmatic and democratic approach to negotiating health resource allocation and rationing. Organizations need enterprise content management (ECM) to play a more important role in business initiatives as video, social networking, analytics and other trends place greater demands on ECM applications. Gartner has revised its Maturity Model for ECM to reflect these new demands. Enterprise architects, information managers and leaders of ECM initiatives can use the model to assess where their organization is today and what level of maturity it must reach to achieve its business goals. Key Findings Gartner's Maturity Model for (...) ECM defines five levels of increasing ECM maturity: Initial (Level 1), Opportunistic, Organized, Enterprise and Transformative (Level 5). ECM maturity is about achieving the right level of ECM capabilities for a particular organization, and optimizing its investment. The majority of organizations are at the Opportunistic and Organized levels. At first, information governance occurs within the scope of tactical content management projects, which typically span two or three information silos. People perform governance roles informally. At Level 3, the ECM strategy supports organization goals, and ECM leaders use business metrics to measure their program. Recommendations At Level 1, look for areas where content silos hurt the business and operations. Then establish governance to start bridging the gaps. At Level 2, work with business process owners to understand the role of content. Identify areas where ECM and business process management (BPM) suites can be integrated. Computational functionalism assumes a synonymy between abstract functional processes in the central processing unit of a typical digital computer and the human brain, hence the conclusion that an appropriately programmed computer is a mind. Arguably, the point is that neural firings are synonymous with the transfer of electrical currents. Both are accountable and susceptible to a physicalist’s explanation. But, the reason they both worked is ultimately premised upon a causal relationship with nature. However, to understand why nature works raises some (...) problems. Nature is either a self-propelled machine or is propelled by another force. However, this seems to imply that mental states are products of the brain and thus, contra Putnam, that an adequate account of mental states which excludes an implementing organic structure is impossible. To this extent, an attempt is made in the paper to structure a biological-organic program. (...) By this structure, it is identified that mental state is a process of the whole organism which necessarily produces phenomenal experience. BackgroundEthics is the science of moral and ethical rules recognised in human life and attempts to verify what is morally right and wrong. Healthcare ethics is seen as an integrated part of the daily activities of health facilities. Healthcare professionals’ standardisation and uniformity in healthcare ethics are urgent and basic requirements. Therefore, this study aimed to assess the practice of the code of ethics and associated factors among health professionals in Central Gondar Zone public hospitals, Northwest Ethiopia, 2021.MethodsA facility-based cross-sectional (...) study design with a mixed method was conducted on 631 health professionals from Central Gondar Zone public hospitals. For the quantitative part, pre-tested self-administered questionnaires were used, and for the qualitative part, key informant interviews with a semi-structured questionnaire were used. Variables with a p value of < 0.2 in binary logistic regression entered into a multivariable logistic regression, then p value < 0.05 and AOR were used to declare statistically significant variables in quantitative data. A thematic content analysis was used for qualitative data analysis.ResultsThis study revealed that only 286 health professionals had good practice of the code of ethics. Good ethical knowledge, favourable attitude, and satisfaction of health professionals with their jobs were significantly associated with the practice of health care ethics.ConclusionsHealth professionals' overall level of practice of health care ethics in the Central Gondar Zone public hospitals was poor. This necessitates practical training, ongoing follow-up, availing of necessary medical equipment, a smooth working environment, and modification of the recognition system for health professionals. BackgroundAmidst expanding roles in education and policy making, questions have been raised about the ability of Clinical Ethics Committees (CEC) s to carry out effective ethics consultations (CECons). However recent reviews of CECs suggest that there is no uniformity to CECons and no effective means of assessing the quality of CECons. To address this gap a systematic scoping review of prevailing tools used to assess CECons was performed to foreground and guide the design of a tool to evaluate the quality (...) of CECons.MethodsGuided by Levac et al’s (2010) methodological framework for conducting scoping reviews, the research team performed independent literature reviews of accounts of assessments of CECons published in six databases. The included articles were independently analyzed using content and thematic analysis to enhance the validity of the findings.ResultsNine thousand sixty-six abstracts were identified, 617 full-text articles were reviewed, 104 articles were analyzed and four themes were identified – the purpose of the CECons evaluation, the various domains assessed, the methods of assessment used and the long-term impact of these evaluations.ConclusionThis review found prevailing assessments of CECons to be piecemeal due to variable goals, contextual factors and practical limitations. The diversity in domains assessed and tools used foregrounds the lack of minimum standards upheld to ensure baseline efficacy.To advance a contextually appropriate, culturally sensitive, program specific assessment tool to assess CECons, clear structural and competency guidelines must be established in the curation of CECons programs, to evaluate their true efficacy and maintain clinical, legal and ethical standards. Precision medicine (PM) aims to revolutionise healthcare, but little is known about the role religion and spirituality might play in the ethical discourse about PM. This Perspective reports the outcomes of a knowledge exchange fora with religious authorities in Singapore about data sharing for PM. While the exchange did not identify any foundational religious objections to PM, ethical concerns were raised about the possibility for private industry to profiteer from social resources and the potential for genetic discrimination by private health (...) insurers. According to religious authorities in Singapore, sharing PM data with private industry will require a clear public benefit and robust data governance that incorporates principles of transparency, accountability and oversight. IntroductionClinical ethics committees (CECs) support and enhance communication and complex decision making, educate healthcare professionals and the public on ethical matters and maintain standards of care. However, a consistent approach to training members of CECs is lacking. A systematic scoping review was conducted to evaluate prevailing CEC training curricula to guide the design of an evidence-based approach.MethodsArksey and O’Malley’s methodological framework for conducting scoping reviews was used to evaluate prevailing accounts of CEC training published in six databases. Braun and Clarke’s (...) thematic analysis approach was adopted to thematically analyse data across different healthcare and educational settings.Results7370 abstracts were identified, 92 full-text articles were reviewed and 55 articles were thematically analysed to reveal four themes: the design, pedagogy, content and assessment of CEC curricula.ConclusionFew curricula employ consistent approaches to training. Many programmes fail to provide CEC trainees with sufficient knowledge, skills and experience to meet required competencies. Most programmes do not inculcate prevailing sociocultural, research, clinical and educational considerations into training processes nor provide longitudinal support for CEC trainees. Most CEC training programmes are not supported by host institutions threatening the sustainability of the programme and compromising effective assessment and longitudinal support of CEC trainees. While further reviews are required, this review underlines the need for host organisations to support and oversee a socioculturally appropriate ethically sensitive, clinically relevant longitudinal training, assessment and support process for CEC trainees if CECs are to meet their roles effectively. Facing the possibility of a surge of COVID-19-infected patients requiring ventilatory support in Intensive Care Units, the Singapore Hospice Council and the Chapter of Palliative Medicine Physicians forward its position on the guiding principles that ought to drive the allocation of ICU beds and its role in care of these patients and their families.

Media guidelines on safe suicide-related reporting are within the suicide prevention armamentarium. However, implementation issues beleaguer real-world practice. This study evaluated the perspectives of the Malaysian media community, persons with lived experience of suicidal behavior, and mental health professionals on suicide-related reporting in terms of the impact, strategies, challenges, and the implementation of guidelines on safe reporting. Three focus group discussions of purposively sampled Malaysian media practitioners, PLE, and MHP were audio-recorded, transcribed, coded and thematically analyzed. Inclusion criteria were: English (...) fluency, no clinical depression or suicidal ideation, no recent previous suicide attempts or suicide bereavement. Three major themes emerged: Unsafe Reporting; Impact; and Safe Reporting. Most described current reporting as unsafe by being potentially triggering to media users and may contribute to contagion effect. Positive impacts identified included raised awareness toward suicide and its prevention. Unsafe reporting was attributed to inadequate awareness, knowledge, and guidance, lack of empathy and accountability, job-related factors, popularity-seeking, lack of monitoring and governance, and information source with unsafe content. Majority agreed on how suicide stories should be framed to produce a safe report. The media community diverged on how detailed a suicide story should be. Safe reporting challenges included difficulties in balancing beneficial versus harmful details, social media ubiquity and its citizen reporters. Participants suggested these safe reporting strategies: stakeholder engagement, educational approaches, improving governance and surveillance, and guidelines revision. Most acknowledged the relevance of guidelines but were unaware of the existence of local guidelines. Implementation challenges included the dilemma in balancing media industry needs vis-à-vis safe reporting requirements, stakeholder engagement difficulties and social media regulation. There is poor awareness regarding safe suicide-related reporting across all groups. PLE and MHP were negatively impacted by current unsafe messaging which aggravated trauma and grief reactions. Postvention support gaps for mental health professionals were highlighted. Safe reporting promotion strategies should include stakeholder engagement to increase awareness on minimizing Werther and maximizing Papageno effects. Strategic re-examination and dissemination of local media guidelines to address new media issues, and effective surveillance mechanisms, are crucial in sustainable improvement of safe reporting practices. The problem of the nature of mind has lingered for a long time. Generated by the question of whether the mind is an independently existing entity or merely an aspect of bodily events and processes, the problem of the nature of mind has divided Western philosophers into two opposing camps, namely dualism and physicalism. Contemporary discourse of the nature of minds, within the Western philosophical tradition, continues to privilege physicalism over dualism, because it avoids the theoretical impasse engendered by the (...) dualist inability to account for how two radically different entities manage to interact with each other. Although physicalism avoids the dualist pitfalls, it, however, encounters the problem of plausibly accounting for the possibility of conscious experience without commitment to the dualist ontology of a realm different from the body. In this article, we provide an African (Yoruba) perspective to the question of the nature of mind as an alternative to the Western perspective represented by dualist and physicalist theories. We develop a variant of dualism called “contextual dualism,” which accepts the dualist basic tenet of the duality of body and mind but diverges from it by permitting that some physical organs of the body also function in the capacity of the mind. Using ethnological analysis and the Yoruba linguistic hermeneutics as theoretical frameworks, the paper argues that the difference between when a physical organ functions as body and when it functions as mind is revealed in Yoruba language through their contexts of use. The paper concludes that contextual dualism drives a reconciliatory wedge between mainstream dualism and physicalism. (shrink)

Background:Suicide remains an important cause of premature deaths and draws much media attention. However, unsafe reporting and portrayal of suicides by the media have been associated with increased risk of suicidal behavior. Current evidence suggests that media capacity-building could potentially prevent suicide. However, there are still knowledge gaps in terms of a lack of data on effective strategies for improving awareness and safe reporting of suicide-related media content. This study aims to investigate the effectiveness of a workshop conducted with members (...) of the media community on the safe reporting of suicide-related content.Methods:An interventional single-arm pre and post pilot study was conducted on a sample of the Malaysian media community recruited through purposive and snowball sampling. The media safe reporting workshop was conducted by a suicide prevention expert with a media industry background. Thirty participants completed a self-reported evaluation questionnaire on their awareness and knowledge of reporting on suicide-related media content; before and after the interventional workshop.Results:There was a significant difference between the total scores before and after the intervention, with a large effect size. Post-intervention scores were significantly improved in 8 items, namely those related to the reporting of: (i) the content of any suicide note; (ii) headlines with methods of suicide; (iii) headlines with the location of suicide; (iv) cases of suspected suicide despite the unconfirmed cause of death; (v) suicide news to cater to readers’ interests; (vi) cause of suicide; (vii) details of the location of suicide; and (viii) the negative impact to media community when reporting suicide stories. In particular, there was an improvement in the majority of items for people from the media community with no lived experience of suicidal behavior.Conclusion:The media safe reporting workshop is a potentially effective intervention for improving awareness and knowledge measures relating to safe reporting on suicide among the media community, with a more pronounced effect in those without lived experience of suicidal behavior. Limitations in the sample size, generalizability, short-term evaluation, and lack of a control group warrant future larger, longer-term controlled, and more representative studies. (shrink)

The growth of positive psychology has birthed debate on the nature of what “positive” really means. Conceptualizations of positive attributes vary across psychological perspectives, and it appears these definitional differences stem from standards for “positive” espoused by three normative ethical frameworks: consequentialism, deontology, and virtue ethics. When definitions of “positive” do not align with one of these ethical schools, it appears researchers rely on preference to distinguish positive attributes. In either case, issues arise when researchers do not make their theoretical (...) alignment explicit, leading to value-laden, often subjective criteria being smuggled into science as a description of what is positive. To foster a deeper critical understanding of the different approaches, we examine how these conceptual definitions of positive attributes (mis)align with their ethical traditions or fail to align with an ethical school. This article describes some potential uses of Bayesian estimation for time-series and panel data models by incorporating information from prior probabilities in addition to observed data. Drawing on econometrics and other literatures we illustrate the use of informative “shrinkage” or “small variance” priors while extending prior work on the general cross-lagged panel model. Using a panel dataset of national income and subjective well-being we describe three key benefits of these priors. First, they shrink parameter estimates toward zero or toward each (...) other for time-varying parameters, which lends additional support for an income → SWB effect that is not supported with maximum likelihood. This is useful because, second, these priors increase model parsimony and the stability of estimates and thus improve out-of-sample predictions and interpretability, which means estimated effect should also be more trustworthy than under ML. Third, these priors allow estimating otherwise under-identified models under ML, allowing higher-order lagged effects and time-varying parameters that are otherwise impossible to estimate using observed data alone. In conclusion we note some of the responsibilities that come with the use of priors which, departing from typical commentaries on their scientific applications, we describe as involving reflection on how best to apply modeling tools to address matters of worldly concern. In the spirit of Von Neumann and Morgenstern (Theory of games and economic behavior, Princeton University Press, Princeton, 1944), we introduce a notion of network stability. We study the structure of stable economic networks and their associated stable payoff allocations by analyzing the conditions under which complete networks and star networks (both with desirable property of inclusiveness) are stable. We also address conditions for existence and uniqueness of stable set of networks.

The progress of electronic health technologies and biobanks holds enormous promise for efficient research. Evidence shows that studies based on sharing and secondary use of data/samples have the potential to significantly advance medical knowledge. However, sharing of such resources for international collaboration is hampered by the lack of clarity about ethical and legal requirements for transfer of data and samples across international borders. Here, the International Clinical Trial Center Network reports the legal and ethical requirements governing data and sample exchange (...) across four continents. The most recurring requirement is ethical approval, whereas only in specific conditions approval of national health authorities is required. Informed consent is not required in all sharing situations. However, waiver of informed consent is only allowed in certain countries/regions and under certain circumstances. The current legal and ethical landscape appears to be very complex and under constant evolution. Regulations differ between countries/regions and are often incomplete, leading to uncertainty. With this work, ICN illuminates the unmet need for a single international collaborative framework to facilitate DSE. Harmonising requirements for global DSE will reduce inefficiency and waste in research. There are many challenges to realising this ambitious vision, including inconsistent terminology and definitions, and heterogeneous and dynamic legal constraints. Here, we identify areas of agreement and significant difference as a necessary first step towards facilitating international collaboration. We propose the establishment of a working group to continue the comparison across jurisdictions, create a standardised glossary and define a set of basic principles and fundamental requirements for DSE. Developing self-report Likert scales is an essential part of modern psychology. However, it is hard for psychologists to remain apprised of best practices as methodological developments accumulate. To address this, this current paper offers a selective review of advances in Likert scale development that have occurred over the past 25 years. We reviewed six major measurement journals between the years 1995–2019 and identified key advances, ultimately including 40 papers and offering written summaries of each. We supplemented this review with an (...) in-depth discussion of five particular advances: conceptions of construct validity, creating better construct definitions, readability tests for generating items, alternative measures of precision [e.g., coefficient omega and item response theory information], and ant colony optimization for creating short forms. The Supplementary Material provides further technical details on these advances and offers guidance on software implementation. This paper is intended to be a resource for psychological researchers to be informed about more recent psychometric progress in Likert scale creation. (shrink)

Whether due to simplicity or hypocrisy, the question of access to patient data for biomedical research is widely seen in the public discourse only from the angle of patient privacy. At the same time, the desire to live and to live without disability is of much higher value to the patients. This goal can only be achieved by extracting research insight from patient data in addition to working on model organisms, something that is well understood by many patients. Yet, most (...) biomedical researchers working outside of clinics and hospitals are denied access to patient records when, at the same time, clinicians who guard the patient data are not optimally prepared for the data’s analysis. Medical data collection is a time- and cost-intensive process that is most of all tedious, with few elements of intellectual and emotional satisfaction on its own. In this process, clinicians and bioinformaticians, each group with their own interests, have to join forces with the goal to generate medical data sets both from clinical trials and from routinely collected electronic health records that are, as much as possible, free from errors and obvious inconsistencies. The data cleansing effort as we have learned during curation of Singaporean clinical trial data is not a trivial task. The introduction of omics and sophisticated imaging modalities into clinical practice that are only partially interpreted in terms of diagnosis and therapy with today’s level of knowledge warrant the creation of clinical databases with full patient history. This opens up opportunities for re-analyses and cross-trial studies at future time points with more sophisticated analyses of the same data, the collection of which is very expensive. (shrink)
